Tomatoe Seedlings Need Help With Diagnosis

Hello all, hoping someone can identify what is going on with my seedlings. The past two years after about 3 weeks of growth, my true leaves start to fold and round spots appear on the under side of the leaves. After a few days the leaves and stems fall off and the seedling dies. I use sterilized medium. Hoping someone can identify the issue so I can correct the problem, see attached image, thx!

This is not a condition I am familiar with. The blotchiness in the second picture makes me think some kind of pest. However, if the problem keeps occurring at about the same timeframe (3 weeks of growth) then I might instead suggest it could be a nutrient issue. You say you use a sterilized media? Can you provide some more details on this? Do you include any fertilizer or compost in the media? If so, what/how much? If not, when and how do you feed them?

What is your watering source (eg: well water, city water, distilled, etc)
